Meningococcal Booster (MCV4) Requirement
All students entering or transferring into the 11th grade will need proof of a meningococcal booster (MCV4) unless their first dose was received on or after their 16th birthday. Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord. Meningitis can cause shock, coma, and death within hours of the first symptoms. To help protect your children and others from meningitis, Georgia law requires students to be vaccinated against this disease unless the child has an exemption.
How Can My Child Get the Vaccine?
When you take your child to their annual wellness visit, please get their vaccination and have the doctor, health clinic, or nurse, print the Certificate 3231 for you. You can go to any Georgia health clinic even if it is not in Fulton County. You can also get the vaccination at a pharmacy (Kroger, Publix, Walgreen’s, etc.). Please call and check with your pharmacy first to see if they offer it.
How Do I Submit My Updated Form?
Please complete the 11th Grade Immunizations Records Form by visiting https://form.jotform.com/232665890488068.
You can upload your Immunization Certificate 3231 at that time.

When Should My Child Get the Vaccine?
Wait until your child turns 16 to get the vaccine but schedule your appointment ahead of time. Do not wait until the last minute to take care of this booster shot. There are students in kindergarten, 7th grade, and 11th grade throughout Fulton County trying to fit in their immunizations all at the same time during the audit period. Sometimes families cannot get an appointment scheduled by the deadline due to high demand.
